Member Travel Reimbursement Form

Members attending the co-operative meetings in May and November in person are able to claim a $20 reimbursement for travel costs incurred (1 per household).

Please download and complete this form copies will be available at the meeting and completed forms can be provided to Sandy, Sue or Melissa at the meeting.
